Symptom narrative

Pt was given pediatric dose my mistake. Contacted the vaccine manufacturer to discuss recommendations for future vaccinations and any adverse effects to monitor for. Patient was contacted notifying them that the wrong vaccine formula was administered and that patent will need to return to clinic to receive the correct Hep B Adult vaccine. Patient did not report any adverse events and had no questions or concerns.
