Describe Event, Problem, or Product Use Error: Administering Proquad (MMRV) vaccine, when I started pushing the vaccine to administer it immediately started to spray out the side of the cap and cap & needle came unattached from the rest of the syringe. Patient was not harmed. Due to all of the vaccine spraying out the vaccine was repeated. Sterile dilutant syringe used to dilute vaccine and draw back up into to administer lot # was Y015120, this was where the cap came off. Needle lot number for 25 gauge needle was 25D088
