Symptom narrative

Patient had grand mal seizure at home witnessed by his mother, lasting less than 1 minute. This occurred at 5 am in the morning. Pt was stable otherwise, mother denied fever, rash, injection site pain. Neurology Dept called and updated pt's provider via nursing line so they can follow up with patient as needed.

chronic otorrhea, seizure disorder (followed by hospital), mild/moderate conductive hearing loss

Depakote 1000 mg once per day
