Symptom narrative

Patient received Spikevax for 12+ instead of the Spikevax 3-11 year old. The pharmacist and pharmacy team was notified on Tuesday evening that the child received the wrong dose. We called the mother to inform them of incident and ask if the child was experiencing side effects or adverse reactions. Mother reported the child threw up on Sunday evening the day they received the vaccine, headache and sore arm at the injection site.
