Symptom narrative

Patient (6 mo old) was inadvertently given Tdap (Boostrix) instead of DTaP (Infanrix). She had no symptoms or reaction. Parent was informed. Parents had elected to receive vaccine components in individual injections so she received Prevnar-20 and Hiberix at the same time.
