Symptom narrative

At 4pm of the same day of imz patient experienced fever, chills, nausea, severe shaking that caused pain in the neck. Patient went to er to seek care. No localized reaction in the arm. patient was given fluids and kept for observation til 4am. Patient states symptoms went away but had chest pain when he got home that resolved without needing additional care.
