Symptom narrative

Patient felt dizzy after receiving the vaccine. Patient was sitting afterwards and lost consciousness for about two seconds then regained consciousness. Patient stabilized to prevent him from falling off the chair he was seated in and then after regaining consciousness was offered water, a fan, and an ice pack. Patient immediately felt better but was monitored for close to 30 minutes before leaving. He seemed coherent and fully able to drive at that point.
