Symptom narrative

Client's mother [withheld name] signed for a TDAP, Hep A, influenza, MMRV, and polio but the TDAP was not administered. Hence, the client ended up getting 4 vaccines instead of 5. Client's mother was contacted by RN and made aware of the error. Mother reported that she will be bring the child in on 10/20/25 for the TDAP dose that was missed.
