Symptom narrative

Patient was seen in office for well check. immunizationas report was reviewed to see what vaccines were due. MMR, DTAP, Hep A were marked as due. Provider discussed with parents and obtained consent to recieve. Medical assistant admisistered vaccines ordered, then logged into database and found vaccines did not interface over. When she went back to review the chart, she found those exact vaccines were given at previous well check on 7/7/2025. These vaccines were documented appropriately in the medical record but did not interface into the Immunizations registry. This led the provider to believe these vaccines were truly due at the time of visit but were not. Provider called parents and informed them of this error. Parents verbalized understanding and noted no adverse reaction at time of call. A follow up call will be made 10/10/2025 to follow up and check on the patient. Please note, our EMR should interface all documented vaccines immediately after documenting. I have contacted immunization registry to start an investigation as to why this did not interface as it should have, which would have prevented duplicate vaccines to be given to this patient.
