Symptom narrative

vaccine given at 1300. At 1420 pt contacted the office stating she had itching and hives. She had taken 50 mg of Benadryl 10 min prior. No resp symptoms. Discussed with PCP who recommended pt go to ER. Pt went to Hospital.

overweight, major depressive disorder, back pain, MASH, PTSD

ventolin inhaler, slynd, probiotic, lyrica, zofran, nicotine patch, MVI, magnesium, lorazepam, hycosamine, claritin, flonase, pepcid, nexium and digestive enzyme
