No effects reported; VAQTA was inadvertently administered to a 6 day old patient; This spontaneous report was received from a Physician and refers to a 6-day-old patient of unknown gender. The patient's medical history, concurrent conditions, concomitant therapies, drug reactions or allergies were not reported. On 19-Sep-2025, in the morning, the patient was inadvertently vaccinated with Hepatitis A Vaccine, Inactivated (VAQTA) 0.5 mL (lot #Z005472, expiration date: 08-Jul-2026) (route of administration, anatomical location not provided) as prophylaxis, at the age of 6 days-old. It was reported that it was due to the medical assistant had accidentally gone into the wrong patient room. No effects reported.
