4 month old patient was accidentally given Kinrix (DTaP+IPV) on 10/10/25, which is approved for children 4-6 years of age and not for a 4 month old patient. Patient's mother and PCP notified on 10/10/25 immediately after error occurred. No known side effects or adverse reactions noted in patient.
