Symptom narrative

Yesterday when I was doing my monthly reports for the Department of Health I had to do immunization reconciliation. I found 2 entries for this patient. I reviewed them and I found that patient received Twinrix (which is adult dose Hep A and Hep B combination) and Pediarix (which is DTaP, Hep B and IPV pediatric combination) on 9/18/25 as part of his 1 Year Well-Child Visit. 4 issues I found: 1. Pt received an Adult dose of Hep A/Hep B combo (Twinrix) instead of a pediatric dose. 2. Pt received 2 Hep B injections at one time. 3. Pt received Dtap early. According to the Child and Adolescent Immunization 2025 Schedule, pt was not due for Dtap dose #4 until 15 months of age. According the to database 4. Pt received IPV dose #4 but this dose is not due until pt is age 4-6 yrs according to the Child and Adolescent Immunization 2025 Schedule. I venture to say that this dose will need to be repeated later when pt turns 4-6 yrs.
