Symptom narrative

I was employed by the hospital for only 2 months at the time, where I was given the shot in my arm (heart side).I left 20 min later.6 min into my drive home I felt a "POP" at tge top left side of my head then a warm feeling rushed over my body from head down to my toes.Immediately after that I got the worse pain in my neck up into my head.My entire left side went numb like it was asleep.I kept speaking out loud and checking in the mirror for signs of stroke but no symptoms like drooping of face, or speech issues. My heart raced and Time seemed to go in slow motion.A 9 mile ride felt like eternity to get home.

Current illness

Only the ones listed in item 12 "prior" to receiving the mandated vaccine.

Medical history

Hypothyroidism, arthritis, depression prior to and ongoing. After the shot, I have continously (over last 4 years) aquired/acquiring new serious health concerns that are not pin pointed to any specific diagnosis. Labs are all over the place, symptoms constantly change, and I fear these changes is from my body attacking itself and I am dying.

Other medications

Levothyroxine, aderall (amphetamine salts), multi vitamin, tramadol

Allergies

Penicillin, MMR vaccine

Lab data

None done at time. New employer no insurance.but documented at work
