Symptom narrative

Patient reported that her paperwork stated she received Prevnar 20 twice from Pharmacy in September. After investigating, it was found she received a dose on 9-4-25 and on 9-24-25. On 9-24-25, the patient requested for the RSV vaccine but received Prevnar 20 in error. Patient reported this on 10/06/2025.
