Symptom narrative

Patient received the Arexvy on 9/14/2024. It was not registered with vaccine registry properly. Upon checking vaccine registry, patient's record did not show Arexvy was administered. We gave the patient another dose of Arexvy on 10/11/2025. No side effects were reported.
