Patient was intended to receive the Measles, Mumps, and Rubella (MMR) vaccine and a separate Varicella vaccine, in addition to the Hepatitis A vaccine, which he received correctly. However, our medical assistant mistakenly administered the MMRV vaccine, which combines MMR and Varicella, along with a separate Varicella vaccine. After finding out about the error, our clinical supervisor consulted the CDC for guidance. The primary concern is the administration of the MMRV vaccine at 12 months of age due to a slightly increased risk of febrile seizures compared to separate MMR and Varicella vaccines given at this age. This may occur up to 10-14 days after administration. The Academy of Pediatrics (AP) typically recommends delaying the combined MMRV until 4-5 years of age to mitigate this risk. There is no significant concern regarding the administration of two Varicella vaccines, as this does not increase reactogenicity or cause additional problems. The patient's mother, a nurse, expressed understanding and appreciation for the transparency, and stated she would monitor Patient's for fevers. I advised to use Ibuprofen and give it for temperatures above 100.4 rectal and inform us.
