Symptom narrative

Site: Bruising at Injection Site-Severe, Site: Swelling at Injection Site-Severe, Additional Details: Gave patient immunization in the left deltoid muscle. After removal of the injection, injection site bled. Once blood was cleaned a "bubble" formed at the site of injection. Patient was instructed to sit for 15 min after vaccine. Patient did not experience any allergic reaction. Patient did not feel signs or symptoms of anaphylaxis. Called patient from home two hours later. Site did not get worse and patient has felt no reactions to the vaccine.
