Symptom narrative

Site: Itching at Injection Site-Mild, Site: Redness at Injection Site-Mild, Site: Swelling at Injection Site-Medium, Additional Details: Injections for three vaccines given on 10/2/25. No issues noted until 10/6/25 where a rash had appeared, about 2 inches wide that is raised/red/warm/itches. She did apply an ice pack and that helped but it came back once ice was stopped.
