Symptom narrative

Site: Pain at Injection Site-Severe, Site: Shoulder Joint Injury (prolonged pain, tingling, etc.)-Severe, Systemic: Weakness-Severe, Additional Details: Pt received vaccine on09/09/2023 , came to see me on 09/23/2025 with arm pain and said she cant raise her arm. Puncture mark was black on her left deltoid, slightly to left of center. I told her to see her doctor right away. Dr ordered prednisone and antiinflammatory with gabapentin but she could not tolerate gabapentin. She came to see me on Oct 7 and said she still has bad pain and cant lie on her arm.
