Systemic: neck pain with loss of range of motion on right side-Medium, Systemic: Headache-Severe, Additional Details: Patent called on 10/6/25 to report headache over the weekend and neck pain, mostly on right side, which was side with covid vaccine. Recommende pt call primary. Primary reached patient on 10/7/25 and recommened urgent care visit. Neurological exam was normal, headache was decreased but there was some decrease of range of motion in neck. Cyclobenzaprine prescribed. Pt report doing better today.
