Symptom narrative

one week after vaccination, patient noted macular rash at the right hand, right thigh, and right foot that progressed to a papular size. one of the sites produced a vesicle and the rash resolved over the course of two weeks. there was crusting on the one vesicle at the right hand. no other vesicles noted at the time. macular papular rash returned on 10/11/2025, without noted vesicles. no fever or chills noted.
