Symptom narrative

Patient sitting upright on patient bed with mother by her side. HPV vaccine administered into right deltoid. Patient immediately became pale in her face and had brief syncopal episode lasting no more than 5 seconds. Mom able to guide her shoulders down to bed as needed. Patient immediately alert and oriented after coming to. Water given and monitored for 15 minutes. Patient able to safely ambulate herself from office.
