Symptom narrative

Patient received vaccination and stated that she was going to sit here for a little while. She started to feel like she felt jittery and asked for water. Gave her sip of water and went to get blood pressure monitor. Upon returning she was slumped over in the chair, but we were able to arouse her. We called 911 and checked her BP (62/38 and 58/42). This took place over 15 minutes.
