Symptom narrative

Employee Health RN (reporter) asked employee if she needed a flu shot this year. Employee consented to receive flu shot. Flu shot was administered without issue. Post flu administration (30 minutes later) employee notified nurse that she forgot she had already been given the flu vaccine in September. No signs or symptoms of adverse reaction.
