In JUN-2025, the patient received Shingrix. In JUN-2025, an unknown time after receiving Shingrix, the patient experienced incomplete dose administered (Verbatim: received "half a dose" of SHINGRIX instead of the full dose) and inappropriate age at vaccine administration (Verbatim: Patient was 31 years). Additional Information: GSK receipt date: 24-SEP-2025 Physician stated that patient received half a dose of Shingrix instead of the full dose, she did not know if that meant just that patients received just the adjuvant or half of the reconstituted vaccine.
