Symptom narrative

Patient reports hives, facial and throat swelling in evening of 10/08/2025. Pt. did not request medical until following morning. Pt. seen at 0700 on 10/09/2025 by medical. Hives were present at that time with no respiratory difficulty , swelling etc.

Hyperlipidemia, Asthma, and Anxiety

Other medications

Prazosin 2mg 3 caps at bedtime. Vistaril 50mg 2 caps at bedtime. Norvasc 10mg 1 QD. Lyrica 300mg 1 @ noon and bedtime. Clonidine 0.1mg 1 QD. Nasacort AQ. Lipitor 40mg 1 at bedtime. Flovent MDI, Albuterol MDI
