Symptom narrative

Per provider's documentation, the patient received the HPV vaccine on the left deltoid, she then developed pain and swelling to her left deltoid area and swelling to her left deltoid and left axilla. The provider advised the patient to be seen in the ED for an ultrasound and evaluation for further treatment.
