Symptom narrative

Client received flu vaccine on 10-01-25. It was reported to this nurse by several people in the community that client became unwell and running fever approximately on 10-6-25. Fever started out at 101 degrees and got as high as 104 degrees. Client was hospitalized at (name withheld) Hospital on approximately 10-07-25 and was placed on "life support". Life support was removed on 10-08-25, client passed away on 10-09-25 from a possible stroke.
