Symptom narrative

RotaTeq was inadvertently administered by the Medical Assistant assigned to this patient who is over the age of 8 months. The patient is currently 15 months on date of administration. At this time no adverse effects have occurred. The MD that saw the patient spoke with the parent about the potential side effects and the parents will call or come back into the clinic if side effects occur.
