10 year old patient here for a well child check visit, seen by rendering provider. The provider ordered the vaccines to administer during the visit: Influenza, Polio and Tdap. the MA had the vaccines dispensed from AccuVax, did not scan the vials and went to RN to verify the vaccines. They were verified by the RN and unfortunately not caught the wrong vial was dispensed by the MA. MA administered the vaccines then had to take her lunch right after. Her colleague helped input the vaccines that were administered and realized the wrong vaccine was dispensed. The colleague informed me, I spoke with the family about the incident. No harm done since the vaccine administered was for someone under the age of 7, I asked the family if they wanted to wait for the providers recommendation or give them a call. The family decided for me to call them at a later time and left clinic by 12:29pm, the rendering provider still needed to finish with her morning clinic but was alerted of the incident. She said once done with her telehealth visits she will look for the next appropriate interval to get the correct dose administered.
