Symptom narrative

No additional AE; a 21 year old female patient was inadvertently administered the PROQUAD vaccine.; This spontaneous report was received from a medical assistant and refers to a 21-year-old female patient. The patient's medical history, concurrent conditions and concomitant therapies were not reported. On 19-AUG-2025, the patient was inadvertently vaccinated with measles, mumps, rubella and varicella (oka-merck) virus vaccine live (PROQUAD) injection, lot #Z008499, expiration date: 03-NOV-2026, for prophylaxis (dose, route of administration and anatomical location were not reported). The vaccine was reconstituted with sterile diluent (lot# and expiration date were not reported). The patient was intended to receive measles vaccine live (schwartz) (+) mumps vaccine live (rit 4385) (+) rubella vaccine live (wistar ra 27/3) (PRIORIX) (Wrong product administered). No additional adverse event was reported.
