At the time of adverse event, office internet was out of service. Child seen for illness and offered to provide flu vaccine. Mom agreed & signed consent. When EMR available (prior to family departing office) error was noted and family informed of error. Vaccine was noted and family informed of error. Vaccine had been provided 9/16/25 (Visit completed with interpreter).
