Symptom narrative

Vaccines were administered about 2:20 Pm on 10/14/25. Patient noted sore injection sites right & left arms around 11pm. Patient was achey and sleepy and did not get out of bed on 10/15/25. At 10am on 10/16/25 patient has chills and headache and vomited during the night. Patient is expected to stay in bed and will miss a scheduled wound care doctor appointment.
