Symptom narrative

patient signed up for the flu vaccine online and completed the consent to receive it. She had received the same vaccine in September 2025. The verifying pharmacist did not catch the duplicate and the high dose flu vaccine (for > 65 yo) was administered. Patient was counseled (as well as husband) on the possible side effects. Pharmacist manager followed up 48 hours later and patient stated she did not experience any adverse effects from the vaccine and was "doing well"
