Called patient and discussed reaction to Covid shot. He stated last year that he got both the influenza and covid the same day, and the following day he fainted and passed out the following day of getting the vaccine. This year he stated that he received the flu shot 2 days ago, and had no reaction so came in yesterday to get the Covid vaccine. He stated during the night he didn't feel good, shoulder hurt and this am, he stated he loss consciousness, vomited and feels good now. He stated his wife is a nurse and was there to witness the episode. Discuss any other symptoms concerns and he stated he feels good. Encouraged them to call their primary provider or to come in to be evaluated. They stated they will call their PCP first and then come in to office if needed.
