Symptom narrative

Flu shot administered with Employee Health on 10/2. On Sunday 10/5 stated rash began on right arm and moved to left arm. Took OTC Benadryl with positive resolution of symptoms. Stopped taking OTC med and rash returned on 10/9, this time on back, chest, face, neck, etc. Employee was sent to Urgent Care for evaluation and worked up for allergic reaction, associated with flu shot. Was given Prednisone and Benadryl
