Symptom narrative

Patient came to charge nurse and reported that post T-DAp injection given by, CMA was not given. Patient stated that the needle was "a big needle and barely placed in my arm". When patient came to CTL, pt had visible liquid down her arm from under the band-aid that was in place as well to her fingers. Pt Significant other also spoke for pt account of events. CTL then administered another TDAP per patients request. Manager notified of events.
