Patient received two combination vaccines (Pediarix and Pentacel) in error during a well-child visit. The intended vaccines were Pediarix, Hib, and PCV20. Pentacel was administered instead of Hib, resulting in duplicate DTaP and IPV doses. The error was discovered shortly after administration. The provider was notified the same day. Parent was not notified until Monday, October 13, three days later. The child reportedly had no fever but was fussier than usual over the weekend. No additional symptoms or complications were observed. No medical treatment was required. Patient monitored at home with no adverse outcome.
Seen in clinic on 09/08/2025 and diagnosed with bilateral otitis media. Completed a 10-day course of amoxicillin. No current illness at time of vaccination.
