Symptom narrative

Seen at ER on 8/26/2025 parents noted patient feeling warm but actual measured temp was not a fever. Diagnosed with viral illness, sent home with plan to alternate antipyretics. Around 7:00pm parents woke to patient shaking in bed, noted to have full body shaking, eyes rolling back, mouth frothing. Episode was about 2 minutes with following post-ictal period ~ 15min. Family called EMS, re-presented to ER and was febrile again but otherwise baseline. Patient then transferred to Hospital for further management. Parents reported some congestion but otherwise no URI symptoms, no known sick contacts, no vomiting, diarrhea, abnormal urine or rashes. No previous history of seizures and no family history of seizures or epilepsy. At hospital patient had a reassuring exam and returned to baseline, though slightly tired in setting of fever. No focal findings on exam to raise bacterial source of infection, making viral illness most likely cause at this time. Provider discussed at length pathophysiology and course of febrile seizures as well as anticipatory guidance related. Patient was discharged on 8/27/25. Later on 8/27/25 in the evening patient re-presented at Hospital due to persistent fevers. On exam patient appeared tearful, however not in acute distress. No cough, rhinorrhhea, rash, erythema of the throat or abnormality in the ear canal was appreciated on exam. When first brought to emergency department patient had a fever of 101.1, was given Motrin, and fever decreased to 98.1. Differentials include but are not limited to benign reaction to vaccines, URI and new allergies other differentials that were less likely include possible bacterial infection or UTI, due to patient's benign physical exam. Patient was interactive, tolerating PO, non-focal exam, discussed with family at length and discharged on 8/28/2025.
