On 30-SEP-2025, the patient received Shingrix. On 30-SEP-2025, an unknown time after receiving Shingrix, the patient experienced inappropriate preparation of medication (Verbatim: pharmacy intern failed to swab / cleanse the stopper of the adjuvant component to Shingrix and adjuvant was not administered) and inappropriate dose of vaccine administered (Verbatim: pharmacy intern failed to swab / cleanse the stopper of the adjuvant component to Shingrix and adjuvant was not administered). The adjuvant was not used to administer the vaccine to a patient. The lot number of the adjuvant was 2XB45, with an expiration date of 26 April 2027. The lot number of the antigen was 2A3HY.
