Patient was in office for their 15 month physical appointment date 10/14/2025 and received two vaccines on each thigh. Mother states no symptoms or reactions late evening. However, she did see a rash the next day during bath time 10/15/2025 at 11:45am which looked like a rash, small bumps located on stomach, both legs, back and arms. No fever or other symptoms at the time. Mom applied calamine ointment to the areas. She did call and came in to our facility for a sick appointment 10/16/2025 patient still had rash with no spreading , no pain and no fever and slight decreased appetite at the time of observation. Mother was advised by our NP to continue with cream and give benadryl. Spoke to mom 10/17/2025 patient is doing well still with no symptoms and rash is still the same.
