Symptom narrative

The patient came to pharmacy and I could not find him in our vaccine database-the patient has a complex name and after numerous searches ( approx. 8 times) with different name combinations and Date of Birth search- I could NOT locate. The patient advised he had not received vaccine with last 10 -11 years- SO i proceeded to vaccinate. Post vaccination after attempting Create a NEW Patient file in database - his name finally appeared ! I then found out he did received a Tdap with the last 10 years ( 5/2017) -approx. 8 and 1/2 years since last one ! SO the dose I gave did not meet the " every 10 year" booster period.
