Symptom narrative

Patient called 10/7 25 at 6:50pm regarding getting help to cover injury related to immunization on 9/24/25. Initial symptoms were soreness in arm for 24 hours, pain in shoulder area after the 48 hours, sharp pain in arm with movement. He said we went to md regarding the incidence and md recommended he takes NSAIDs and gets a muscular ultrasound done to check of everything is in order and no injuries regarded to rotator cuff (SIRVA).
