PATIENT CALLED THE PHARMACY ON 10/19/25 AND REPORTED THAT HIS ARM HAS BEEN SORE WITH LIMITED MOBILITY SINCE THE TIME THE VACCINE WAS ADMINISTERED ON 10/16/25. HE SAID THAT HE IS UNABLE TO MOVE HIS LEFT ARM ABOVE THE LEVEL OF HIS SHOULDER. HE REPORTS THAT HE BELIEVES THE VACCINE WAS ADMINISTERED TOO HIGH ON THE SHOULDER.
