Symptom narrative

Systemic: Fainting / Unresponsive-Mild, Additional Details: After patient received the vaccine, she walked out to her father in the waiting area. Patient's father stated that patient was standing up while talking to her mother and suddenly fell down and fainted for a short amount of time. When I was notified that the patient had fainted, she was alert. I asked her parents to let her sit down on the chair and I fanned her. I offered the patient a bottle of water. The patient and family left after 15 minutes of observation.
