Symptom narrative

Systemic: wrong vaccine not side effects-Mild, Additional Details: Patient was due to receive covid vaccine (5-11 y/o). Parents were also receiving covid vaccine. Both adult covid vaccines were pulled and ready for administration. Accidently gave child adult covid vaccine instead of pediatric strength. Parents made aware of event and told to monitor child for side effects. Pediatrician will be notified once office opens and apology was given to parents.
