Symptom narrative

Site: Pain at Injection Site-Mild, Systemic: Chest Tightness / Heaviness / Pain-Mild, Systemic: Dizziness / Lightheadness-Mild, Systemic: anxiety-Mild, Systemic: Flushed / Sweating-Mild, Systemic: Hypertension-Mild, Additional Details: Patient came back to the pharmacy and let me know that she wasn't feel well after vaccine and thought she was having a reaction. She said that her chest felt strange like she could feel vaccine traveling through her body. She said she feels similar to when she has an epinephrine injection at the dentist. She told me she has a history of anxiety and often feels this way after a vaccine. She did not document that on intake questionaire. I checked her blood pressure it was 169/92 pulse 103.
