Symptom narrative

Site: Pain at Injection Site-Mild, Systemic: instructed to seek medical care-Mild, Additional Details: The patient received 5ml of IPOL vaccine instead of 0.5ml in error. Immunizer was R.N. who drew up the dose & administered the dose. Once the error was identified, RN contacted the manufacturer (Sanofi) who advised she inform the patient to seek medical care. RN contacted the patient via a phone call & advised he seek medical care. The pharmacy manager also followed up with the patient via phone call to check in reiterate the manufacturers recommendation to seek care.
