Systemic: Dizziness / Lightheadness-Severe, Systemic: Exhaustion / Lethargy-Severe, Systemic: Hypotension-Severe, Additional Details: Discussion with pt and companion revealed that pt had recently been discharged from rehabilitation after back surgery. Upon administration of vaccines, pt stated that he felt weak and unable to rise from chair. After a few moments without improvement, companion asked that we call emergency services. Upon arrival of EMTs it was discovered that pt's blood pressure was low. He agreed to be taken to local hospital for observation.
